Straight and symmetric legs are one of the most important elements of the visual quality in women. Thickening of the legs due to weight gain most often is the most common aesthetic problem followed by sagging and thinning.

Legs are anatomically examined in three parts including the patella, femur – the upper part of the patella -, and calf – lower part. Aesthetic problems seen in these areas are quite different from each other and should be considered individually.

Main aesthetic complaints regarding the femur include local fat accumulations, round thickening of the femur, and sagging of the inner part of the femur. The treatment to be applied for each complaint will vary in shape and size.

The first option in repair of any fat accumulation or asymmetries in the femur is liposuction. Liposuction is often performed under general anesthesia or sedation and lasts about 1-1,5 hours depending on the width of the operation area. This operation is to remove the fat tissues causing asymmetry and thickness by cannulas inserted from a few 2 mm holes. Generally no post-operation pain will occur, and patients get to the feet 2 hours after the operation and stay 1 day at the hospital.

The treatment in repair of the sagging of the inner part of the femur is femur lifting combined with liposuction. The main purpose of femur lifting operations is to remove the saggy excess skin. This operation is also often performed under general anesthesia and the fat tissue accumulated in the inner part of the femur will be removed by liposuction as the first step. Subsequently, partial or full femur lifting will be applied in parallel with the severity of the sagginess. In partial femur lifting operation, cuts will be made on the groin (the junction point of the abdomen and the legs) and fully hidden inside the underwear. ın full femur lifting, an additional scar going down the knee will be left. Partial femur lifting mostly provides a vertical lifting while full femur lifting ensures an apparent tightening in the vertical axis. In both types of femur lifting operations, the patient can get to the feet and walk the same day and will be discharged the next day. Patients can go back to their daily life in about 4 days and take long walks as of the end of the 1t week, and go running and do other sports activities in 15 days.

The most common complaint about the knee area aesthetic is mild sagginess and fat accumulation around the knee. Particularly the fat accumulated in the inner part of the knee can cause major aesthetical concerns when skirt or tight clothes are worn. The first option to treat such fat accumulations and mild sagginess is liposuction. Patients can fully eliminate this complaint in about 30 minutes by this operation to be performed under sedation with local anesthesia, and discharged the same day.

Unlike the other areas, the most common complaint in the calf area is having thinner calves compared to the upper leg. There are two options to solve these complaints. The first and most preferred one is the use of silicone prostheses. This operation that can be applied under general or spinal anesthesia allows inserting prostheses from a 4-5 cm cut on the back of the leg and the patient will get to the foot and walk the same day following the operation. Mild pain can be felt when walking for 2-3 days after the operation and you can go back to your sports activities in 3 weeks. Since prostheses are inserted at a certain depth from the skin, they will not be palpable in future. Damage or deformation of prosthesis is seen much more rarely in the long term since mostly prostheses made of hard silicone are preferred. Another alternative to thicken that area is the fat graft application. Especially in patients with fat accumulation in other parts of the body to be removed by liposuction, such fat can be thinned and used to thicken the legs in the same session.

Another complaint regarding leg aesthetics is thick ankles. Fat accumulations other than bones, skin and tendon can make ankles look thicker and rough than they actually are. The best option for treatment of this area is also liposuction performed under local anesthesia + sedation, and the patient can be discharged the same day. There is no surgical treatment for thickness resulting from bones, skin, tendons other than fat accumulations.
